The overall goal of our case management services is to assist individuals and families in accessing support services and community resources that may meet basic needs, provide safety, stability, and restore social, academic, and/or vocational functionality.
Case Management is a service provided separately but in collaboration with counseling and coaching services.
Case management for youth ages 5-17, may be covered by Husky.

Rooted in the client-centered relational approach, Monarch Therapeutic Services' Case Management services SUPPORT the client's development and use of new skills, knowledge and wisdoms, self-advocacy and self-sufficiency to help meet basic needs and restore healthy functioning.
Monarch Therapeutic Services Case Management activities may include:
~ Supporting clients applying for Social Services, Disability, FMLA, etc
~ Collaboration, coordination, and advocacy with other providers, agencies, schools, etc.
~ Assessing needs and appropriate interventions & resources to help client address needs.
~ Increasing client awareness of interventions and resources accessible to them.
~ In some limited circumstances, direct intervention and support in connecting client to resources.
Examples of Case Management Services:
~ Attend school meetings
~ Attend DCF meetings
~ Coordinate with other providers
~ Referrals to other providers
~ Referrals to self-help resources.
~ Facilitating connections to community and resources.
